The Residential Health Specialist identifies and connects members with residential providers based on their individual needs. They collaborate with care teams to ensure members receive appropriate support and maintain accurate documentation within the software platform.
Candidates must have a High School Diploma or GED and at least one year of experience working with IDD, TBI, or behavioral health populations. A valid driver's license and residency within the specified Southern Region counties are required.
