The Social Worker will conduct psychosocial evaluations, provide ongoing counseling, and deliver bereavement services to patients and families. They will also participate in community education outreach and provide emotional and spiritual support in accordance with the established plan of care.
Candidates must hold a Master's degree in social work from an accredited institution and possess a current state license. A minimum of 2 years of clinical experience in healthcare or hospice is required, along with the physical ability to perform patient care duties.

Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively
