Provide community-based mental health case management services to youth, adolescents, and young adults. Collaborate with professionals to develop treatment plans and maintain accurate documentation for client files.
Requires a Bachelor’s degree in Social Work or a related field and a valid driver's license with a good driving record. Experience working with youth and adolescents with mental health concerns is preferred.
$20.19 and up Essential Functions: Provide community-based mental health case management services to youth; adolescents and young adults. work closely with other professionals in providing treatment services in the areas of mental health, employment, education, and Housing Have Knowledge of community systems like dependency, foster care and adoption and residential services. provide community-based services develop treatment plans and maintain accurate and current documentation for client files.
Bachelor’s Degree in Social Work or related field required. Experience with youth, adolescents and young adults with mental health issues and concerns preferred. Experience with multi-system youth, adolescents and young adult preferred. Must have excellent written and verbal communication skills. Must have a car and good driving record.
