The Nurse Practitioner or Physician Assistant will provide direct primary care, conduct comprehensive assessments, and manage chronic conditions for residents in senior living facilities. They will also coordinate care with facility staff and families while maintaining accurate electronic medical records.
Candidates must hold a valid Nurse Practitioner or Physician Assistant license and current board certification in their field. A Master's degree in Nursing or Physician Assistant Studies is required, along with strong leadership and communication skills.
Provide high‑quality medical care in SNF and ALF settings Step into existing care teams to ensure seamless patient coverage Adapt quickly to new facilities, workflows, and clinical environments Maintain accurate and timely documentation in accordance with Curana standards Collaborate with facility staff and Curana clinical leadership to support patient outcomes Schedule & Travel Requirements Full‑time, Monday–Friday Assignments may be anywhere within North Carolina Most assignments are within 30 minutes to 3 hours from home; occasional assignments may be farther, depending on coverage needs Providers typically travel Monday morning and return Friday afternoon No weekend coverage required Assignments generally range from several weeks up to 12 weeks Providers receive additional support to account for frequent weekday travel and short‑term facility coverage

Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field Masters Degree in Nursing or Physician Assistant Studies Strong leadership, interpersonal, and problem-solving skills preferred. Must possess excellent written and oral communication skills Must possess traits of punctuality, organizational skills, and ability to function within a team. This role requires travel to and from one or more assigned facilities. Depending on the amount of travel required, Curana may in future determine that a valid driver’s license, current auto insurance, and an acceptable driving record are required.
