The Social Worker provides specialized clinical knowledge and advanced skills for assessment, treatment, patient/caregiver navigation, mental health support, and discharge planning, particularly for transplant patients as mandated by CMS. Responsibilities include attending multidisciplinary rounds, identifying necessary resources, facilitating support groups, and consulting with providers to monitor treatment adherence.
Candidates must possess a Master's degree and hold a Licensed Clinical Social Worker (LCSW) license in the State of Florida, along with passing a Fingerprint Screen. A minimum of two years of post-MSW experience in a related field, including previous hospital setting experience, is required.

The Licensed Clinical Social Worker (LCSW) at Memorial Primary Care supports the Sickle Cell Medical Home Program by providing comprehensive case management and behavioral health services within an integrated primary care setting. This role utilizes advanced clinical expertise to support patients and caregivers with care coordination, mental health and behavioral health needs, crisis intervention, discharge planning, and linkage to community resources. The LCSW also conducts home visits as needed and collaborates closely with the interdisciplinary care team to promote continuity of care, improve health outcomes, and ensure patient-centered support for individuals living with sickle cell disease.
Participation in transplant selection committee meetings.Design and facilitate support groups and other programs.Attend multidisciplinary rounds as mandated by CMS for all Medicare-approved transplant centers and identifies appropriate resources for financial, insurance, transportation, housing, healthcare, mental health, spiritual services, and education about diagnosis and transplantation.Develop behavioral agreements with patient and/or families that addresses adherence and commitment to treatment plan. Assist with patient fund-raising.Assist patient in the comprehension of complex instructions, resolve conflicts, or facilities conflict resolution, have strong organization/ priority setting skills, must be able to learn and apply large amount of technical and procedural information, must be able to multi-task, and must be able to communicate effectively, verbally, and in writing.Interviews and assesses patients and/or family, caregivers, and/or legal representatives. Performs comprehensive psychosocial assessments for pre- post and living donors to determine transplant candidacy and continue to work with patient and family through all phases of the transplant process during inpatient and outpatient and end of life. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.May supervise and/or train new staff, students, and interns.Provides crisis intervention for patients and family members.Consults and collaborates with multidisciplinary transplant providers as mandated by CMS for all Medicare-approved transplant centers to promote, monitor, and evaluate patient's ability to understand and adhere to treatment plan and lifestyle changes. Make appropriate referrals to community agencies, such as DCF, APS, SSA, CMS, disease specific resources as deem necessary.
