The Clinical Pharmacy Technician is responsible for obtaining accurate medication histories from patients in the Emergency Department or inpatient units. They will interview patients and caregivers, verify data with outpatient pharmacies, and document findings in the electronic health record to assist pharmacists.
Candidates must have a high school diploma or GED and at least one year of pharmacy experience. A Pharmacy Technician License is required, and PTCB certification must be obtained within 180 days of employment.
A day in the life of a Clinical Pharmacy Technician at Hackensack Meridian Health includes: 1. Obtains accurate medication histories from patients in the Emergency Department or inpatient units as necessary 2. Interviews patients/patient's caregivers to obtain most precise data about the patients medications and supplements (e.g. name of medication, dose, frequency, route, last dose taken, last time refilled)3. Calls patient's outpatient pharmacy and or healthcare facility to obtain the best possible medication history (as above)4. Utilizes electronic health record to collect patient related information and document work done as necessary5. Communicates all medication discrepancies or complicated medication regimens to registered pharmacist 6. Utilizes available resources to communicate with a translator if necessary7. Communicates with other healthcare professionals as necessary 8. Demonstrates an understanding of and adheres to all medication use and pharmacy policies and procedures. 9. Performs other pharmacy technician duties as assigned.10. If Outpatient Pharmacy Technician is qualified, enter prescriptions into the order entry system.11. Other duties and/or projects as assigned.12. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications: Education,
Required: 1. High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.2. At least one year of pharmacy experience (outpatient or inpatient) required. 3. Effective oral and written English communication.4. Strong ability to multitask.5. Ability and willingness to work cooperatively with others.6. Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms. Licenses and Certifications Required: 1. Pharmacy Technician License.2. PTCB Certification Required within 180 days of start of employment with the following exceptions: a. Hired prior to January 2024 b. Actively enrolled in ACPE accredited school of pharmacy / another allied health program or hired before 1-1-24. Licenses and Certifications Preferred: 1.
