The Community Care Navigator provides mental health and substance use support services to clients within their natural environments, including homes and schools. They act as client advocates, coordinate treatment plans, and provide crisis intervention to ensure clients remain in the least restrictive settings.
Candidates must have a high school diploma or GED and experience working with youth or families. Preferred qualifications include an associate degree or higher and three or more years of experience in the behavioral health field.
Who are you?
You are a caring professional who is dedicated to providing mental health and/or substance use services for your clients. You have a passion for making a difference in your local community by providing community support services for clients while in their natural environment (home, school, community, etc.).
Under the guidance of the Prevention Supervisor, you will serve as client advocate, acting in their best interest at all times. Attend treatment plan development and review meetings and coordinate/monitor the appropriateness of all services. Provide assistance in crisis situations to stabilize client and maintain client in the least restrictive treatment setting. Use crisis assessment tools and coordinate crisis services based on results; coordinate and implement universal screenings within the school systems.
Working in a team-based environment is important to you. While working with the treatment team, you will maintain a strengths-based approach as you work with your clients and families to assist with the retention of dignity and the development of positive qualities in the client.
In this position, you will regularly use various computer apps including Microsoft Office, our HRIS, EHR and other online systems, with multiple logins often simultaneously. You will have the ability to obtain an Ohio driver’s license, maintain auto insurance, earn CPR/First Aid certification, and be willing to submit to background check and fingerprinting.
