The Case Manager serves as a primary support for individuals with mental illness and co-occurring disorders to assist them in living independently. Responsibilities include providing community-based, holistic support and driving clients to appointments and group activities.
Candidates must have a Bachelor's degree in Social Services or Human Services with one year of experience, or a high school diploma with four years of direct mental health experience. A valid driver's license and personal vehicle insurance are required for the role.

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Case Manager III is distinguished from Case Manager II by the additional education, years of experience, and ability to proficiently perform the job functions and meet position standards.
Expected starting wage range is $21.49 - $26.25. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

CARE Court Assertive Community Treatment (ACT) is a Telecare partnership with San Diego County Behavioral Health in response to California’s Community, Assistance, Recovery, and Empowerment (CARE) Act. CARE Court ACT delivers time-limited, community-based, holistic support to people who meet CARE criteria and agree to participate in these services. Our person-centered approach to care meets people wherever they are in their recovery — addressing the complex needs of the individual and connecting them to resources to thrive in their community. Who We Serve: All criteria must be met to be eligible for admittance: 1) Aged 18 years and older, 2) Diagnosed with a schizophrenia spectrum disorder or another psychotic disorder in the same class, 3) Individuals must be experiencing symptoms that are persistent, interfere with daily life, and may lead to a loss of independence, 4) Individuals can not receive voluntary treatment elsewhere, 5) One of the following must be applicable: Individual is at risk of safely interacting with the community without supervision, and their condition is deteriorating, Individuals who need services to prevent relapse or deterioration that would result in disability or serious injury to themselves or others, Individuals who will likely benefit from ACT treatment plan.
