The Intensive Case Manager will assess participant needs, develop treatment plans, and coordinate essential services such as housing, mental health, and substance abuse support. They are also responsible for maintaining comprehensive case records and collaborating with various community partners to prevent recidivism.
Candidates must possess either a bachelor's degree in behavioral health or social science with one year of experience, or a high school diploma with three years of relevant experience. A valid driver's license, personal vehicle, and car insurance are required for travel to jails and client appointments.

The intention of our services is to help people in Los Angeles County who have not been able to get the needed help in terms of Housing, Mental Health, Substance Abuse treatment, and linkages to community resources to help minimize recidivism. The role of the Intensive Case Manager is to help create a supportive network of services and resources for the client to prevent reincarceration and support long term permanent housing goals.
