As a Registered Nurse for Prime Home Health, you will provide compassionate and skilled nursing care to patients on a Fulltime basis in the comfort of their homes.

You will be responsible for assessing patients' needs (OASIS experience mandatory), ability to learn Nestmed AI charting, developing care plans, coordinating with other healthcare professionals, and implementing appropriate interventions to promote optimal health and well-being. Your expertise, clinical judgment, and strong interpersonal skills will be crucial in delivering high-quality care and ensuring positive patient outcomes.

Responsibilities

Patient Assessment and Care Planning

• Conduct comprehensive assessments of patients' physical, emotional, and social needs to develop individualized care plans. Collaborate with patients, their families, and healthcare teams to establish goals and interventions to address patients' health conditions and promote self-management. Regularly reassess patients' conditions, monitor vital signs, and evaluate the effectiveness of interventions, adjusting care plans as necessary. Educate patients and their families on disease management, medication administration, and self-care techniques.

• Nursing
• Care: Provide skilled nursing care including wound care, medication management, IV therapy, catheter care, and other treatments as prescribed. Administer medications and treatments accurately and safely, following established protocols and guidelines. Perform skilled procedures such as venipuncture, tracheostomy care, and respiratory therapy. Monitor and manage symptoms, pain, and comfort levels, implementing appropriate interventions to alleviate distress.
• Coordination and
• Collaboration: Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop and implement comprehensive care plans. Coordinate with the interdisciplinary team to ensure continuity of care and effective communication. Communicate regularly with patients, families, and caregivers to provide updates on the patient's condition, progress, and any changes to the care plan. Coordinate and supervise home health aides and other support staff involved in patient care.
• Documentation and
• Compliance: Maintain timely, accurate and thorough documentation of assessments, care provided, and patient outcomes using Nestmed AI charting tools and Kinnser EHR system. Ensure compliance with all regulatory requirements, agency policies, and standards of care. Complete and submit clinical documentation, including OASIS assessments, within specified timeframes.
