First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant for our family care practice in Manchester, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting.

Experience and Training
Proven experience as a Nurse Practitioner or Physician Assistant evaluating patients of all ages and establishing care plans
A current license to practice as a NP or PA in the state of Connecticut.
Appropriate Board Certification
A commitment to serving diverse and vulnerable populations with compassion and respect.
Excellent communication, interpersonal, and organizational skills.
Connecticut Controlled Substance license and DEA license
BLS Certification through the American Heart Association or American Red Cross.
Pay is based on several factors including but not limited to work experience certifications, etc. Range for this position is $115,000 - $160,000
