Coordinates care management services for family members and caregivers, focusing on high-risk patients and those transitioning from inpatient services. Acts as a liaison between providers and community resources to ensure patient-centered care and reduce gaps in follow-up care.
Requires a high school diploma and 1-3 years of relevant experience, with a bachelor's or master's degree strongly preferred. Experience in health homes or care management and a NYS driver's license are highly desired.
Coordinates and participates in activities related to Care Management services to family members and caregivers.
