• Provide compassionate, hands-on patient care, including end-of-life support Follow individualized care plans and keep documentation accurate and timely Educate patients and families so they feel supported and informed Work closely with an interdisciplinary team to deliver the best possible care Manage pain and symptoms to ensure comfort and dignity Be part of a team that truly supports each other and the patients we serve

• For: Graduate of an accredited nursing program At least 1 year of nursing experience (hospice, home health, or med/surg is a plus) Active LPN license Strong communication and organizational skills Valid driver’s license and reliable transportation
