Provide direct, evidence-based behavioral health care to children and families using the Collaborative Care Model via telehealth. Coordinate care with pediatricians, psychiatrists, and external services while tracking patient progress through validated screening tools.
Requires an active unrestricted clinical license (LCSW, LPC, or LMFT) in specific US states and at least 3 years of experience or 3,000 supervised hours. Must be proficient in evidence-based modalities like CBT and comfortable working in a remote environment with flexible hours.
The Role We’re hiring Behavioral Health Care Managers (BHCMs) to provide direct, patient-facing care to children, teens, and families within the Collaborative Care Model (CoCM). This is a fully remote role where you will: Spend ~75% of your time working directly with patients and families Deliver brief, evidence-based care designed to drive meaningful change in 6–12 months Collaborate closely with pediatricians, psychiatrists, and a multidisciplinary care team
Active, unrestricted clinical licensure as a licensed clinical social worker (LCSW), licensed professional counselor (LPC), and/or licensed therapist (LMFT). Active licensure in one or more of the following states on or before start-date: California Colorado Connecticut Florida Maryland Tennessee Texas Virginia Washington Experience working with children, adolescents, and families 3+ years of experience OR a minimum of 3,000 supervised hours Flexibility with hours/availability, including 2 evenings/week + 2-4 weekend days/month Experience and training in evidence-based modalities (CBT, MI, BA, etc.) Comfort working in a telehealth environment Strong communication, organization, and documentation skills Ability to deliver care with a diversity equity and inclusion (DEI) and trauma-informed lens Nice to Have Experience in: Pediatric primary care or collaborative care (CoCM) School-based or community mental health settings Familiarity with: Measurement-based care Brief, structured interventions Benefits of Joining the Triplemoon Team Fully remote role Meaningful, patient-centered work A collaborative care model that supports you clinically Focus on impact, not just volume Opportunity to help transform pediatric behavioral health Ongoing training and development opportunities.
