The coordinator assists patients in navigating post-acute care to positively impact patient outcomes and referral source satisfaction. This involves integrating evidence-based clinical guidelines to develop patient-centered transition plans that promote quality and efficiency in post-acute care delivery.
Candidates must be graduates of an approved school of nursing, therapy, or social work and must be licensed in the state of practice, requiring a minimum of two years of demonstrated field experience. Essential requirements include understanding Medicare certified home health and hospice regulations and possessing basic technology skills, along with maintaining a valid driver's license and dependable transportation.
Overview As a Clinical Liaison (Care Transitions Coordinator), you will assist patients with the process of navigating post-acute care. Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services.
Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care. Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities
Education and experience, essential Must be a graduate of an approved school of nursing, therapy or social work. Must be licensed in the state where they currently practice. Must have two (2) years of demonstrated field experience. Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred A Registered Nurse RN, Physical Therapist PT, Occupational Therapist OT or Licensed Medical Social Worker LMSW is preferred. Three (3) years of field experience is preferred. Previous experience in home health or healthcare sales is preferred.
