Serve as the primary point of contact for hospice patients and families, delivering skilled nursing care and managing individualized plans of care. Collaborate with an interdisciplinary team to provide physical, emotional, and spiritual support while maintaining accurate clinical documentation.
Requires an active RN license and at least one year of experience in hospice, home health, ICU, oncology, or geriatrics. Must possess a valid driver's license, reliable transportation, and CPR certification.
