Perform daily rounds at multiple post-acute facilities to provide comprehensive wound care and bedside treatment for geriatric patients. Collaborate with facility staff and primary care physicians to develop and implement effective wound care plans.
Candidates must be a licensed Nurse Practitioner or Physician Assistant with a valid driver's license. Ability to lift 50 pounds and travel to various facilities in a personal vehicle is required.
Active state professional license Valid driver’s license Duties: Weekday commitment: No call, evenings, or weekends Deliver wound care at the bedside in the post-acute care setting. Round daily at multiple facilities Responsibilities: Conduct comprehensive patient assessments, paying special attention to factors that increase the risk of wound formation. Collaborate with facility staff to ensure an effective wound care plan. Perform wound debridement and prescribe appropriate wound dressings. Educate nursing staff, and maintain open communication with primary care physicians, families, and clinical teams as required. Order necessary medications and provide clear instructions for their use. Provide quality patient care to the Geriatric patient population.
