Provide hands-on personal care and support services to adult residents in long-term and transitional care units. Responsibilities include monitoring vitals, documenting in electronic medical records, and assisting with daily living activities.
Must be active on the MN Nursing Assistant registry by the start date or enter the training program. Experience as an HHA, PCA, DSP, or in customer service is preferred.
Assists resident based on individual care plan with personal cares such as; bathing, oral hygiene, grooming, toileting, dressing, showers and responds to call lights Nursing Assistant may per community patient care model check and record vitals AM/PM cares; including peri care and catheter care May turn, lift or transfer residents with or without other persons/mechanical assistance Observes residents and reports status to appropriate staff in a clear and concise manner Chart or document in electronic medical record system Seeks to resolve resident and family concerns Receives and gives report to appropriate staff during various times in work shift Other duties such as; order and cleanliness in rooms and work areas, reports malfunctioning equipment to maintenance department, and answers phones in a timely manner Required: Active on MN Nursing Assistant registry by start date If hired into the Ebenezer Nursing Assistant Training program, granted a 4-month grace period from hire date to be active on the MN Nursing Assistant registry Preferred: Ability to work with a diverse and geriatric population Communicate efficiently, with empathy and understanding for residents and coworkers Experience working as a HHA, PCA, DSP, NA, Resident Assistant Work experience in a customer service setting
