We are looking for a passionate and dedicated Cultural Community Health Worker (CCHW) to support and uplift Black, Indigenous, and People of Color (BIPOC) communities. As a trusted connector, you’ll provide culturally responsive outreach, health education, and care navigation, while addressing social determinants of health and advocating for individual and community needs. You’ll meet people where they are—in their homes, neighborhoods, hospitals, and community spaces—to help reduce health disparities, increase access to services, and promote human rights and social justice.
