The RN provides nursing care to hospice patients in their residences and coordinates with an interdisciplinary team. Responsibilities include performing assessments, implementing individualized care plans, and providing education to patients and caregivers.
Candidates must be a graduate of an accredited nursing school with a current RN license and prior hospice experience. A valid driver's license, reliable transportation, and auto insurance are required for home visits.
Registered nurse (Rn) – hospice
The Registered Nurse (RN) provides nursing care to hospice patients in their place of residence and coordinates care with the Interdisciplinary Team (IDT), physicians, patients, families, and referral sources. The RN is responsible for assessing patient needs, implementing the plan of care, and ensuring quality end-of-life care.
Per-Visit Rate and On-Call rate to be discussed upon interview.
