The Permanency Support Services Case Manager is a combination of Post Permanency and Post Adoption Case Management.
The Permanency Support Services Case Manager is a member of the Permanency Support Services program.
The Permanency Support Case Manager will work with the Supervisor of the Permanency Support program to provide information and referral services to post adoption and/or post permanency families seeking services, as needed.
The Permanency Support Services Case Manager will develop and provide outreach to the community to increase awareness of services available to families.
The Permanency Support Services Case Manager will work to maintain an effective working relationship between all Arms Wide programs and other Child Placing Agencies to provide a continuum of service to families granted Permanent Managing Conservatorship (PMC) of children from foster care and/or families who have adopted from foster care - Post Adoption families.
Permanency Support Services seek to strengthen family functioning and to increase the quality of life for all family members.
Casework consists of the planning and coordination of post adoption or post permanency services to a child and their family to help the family and child with promoting safety and security within the family, cope with any history of abuse in the child's background and avoid permanent or long-term removal of children from the family.
The case manager's goal is to help the family remain intact.
