Acts as the primary liaison between the hospice organization and hospital systems to coordinate patient admissions and care. Responsible for assessing patients, collaborating with discharge planners, and ensuring adherence to the hospice Plan of Care.
Requires a minimum of an Associate's Degree in Nursing and a current, unrestricted NC/SC Registered Nurse license. Candidates must have at least 2 years of related experience in direct care for patients with life-limiting illnesses.
