Lutheran Services Carolinas (LSC) is seeking a compassionate Community Support Specialist to provide direct, community-based support to veterans particularly those living with traumatic brain injury (TBI), PTSD, or other disabilities. This role focuses on empowering veterans to build independent living skills, strengthen community connections, and improve overall quality of life through individualized, respectful, and trauma-informed support. Key Responsibilities Direct Support (Activities of Daily Living) Provide one-on-one assistance with activities of daily living (ADLs), including personal hygiene, grooming, laundry, housekeeping, meal preparation, and basic money management Promote independence by encouraging skill development and self-sufficiency Community Engagement Support participation in recreational activities, peer support opportunities, and community events Help reduce social isolation and build natural support networks within the community Care Coordination, Advocacy & Documentation Develop, implement, and monitor individualized service plans based on veteran needs and goals Advocate for veterans’ needs while ensuring dignity, respect, and person-centered care Coordinate and/or provide transportation to medical, dental, behavioral health, and community appointments Maintain accurate and timely documentation, including service notes and progress toward goals Communicate changes in needs or concerns to supervisors and interdisciplinary team members Safety & Crisis Support Assist in developing and implementing crisis response and safety plans Monitor veteran well-being and respond appropriately to safety concerns Collaborate with healthcare providers, case managers, and other service partners as needed Qualifications
