The Community Health Worker serves as a link between healthcare systems and vulnerable populations by conducting outreach and assessments. They coordinate comprehensive community-based services and provide health education to improve member stability and health outcomes.
Requires a high school diploma (degree preferred) and at least 2 years of experience in community health or social services. Candidates must possess a valid California Driver's License and a personal vehicle for extensive travel within the assigned county.
High school diploma or equivalent; Associate’s or Bachelor’s degree preferred Minimum of 2 years of experience in community health, social services, or related field Strong understanding of community resources and social determinants of health Excellent interpersonal, communication, and engagement skills Proficiency in electronic health records and basic computer systems Bilingual skills preferred (Spanish or other relevant languages) Must be proficient in technology, including documentation systems, EHR platforms, and communication tools Ability to document in real-time and manage multiple systems effectively Work Requirements Ability to travel extensively within assigned county Must reside within assigned service area Ability to work occasional evenings or weekends for community events Ability to work in a hybrid, field-based environment Additional Requirements Possess a valid California Driver’s License (Class C minimum) Maintain a personal, operable vehicle for daily business use Maintain current liability insurance that meets California’s minimum legal requirements Must pass a Motor Vehicle Report (MVR) background check prior to employment Compensation & Benefits Hourly Pay: $21.00 – $24.00 per hour Monthly stipend 160 hours PTO + paid sick time 11 paid holidays (including birthday and floating holiday) 4 paid volunteer hours per month 90% employer-paid employee-only medical benefits Flexible Spending Account (FSA) Short-term & long-term disability, AD&D Employee Assistance Program (EAP) 401(k) with company match Professional development opportunities Quarterly in-person events Employee discount programs
