Candidates must hold a BSW or MSW from an accredited school and possess a current, unencumbered Social Worker License. A minimum of one year of healthcare social work experience, CPR certification, and a dependable vehicle are required.

POSITION OVERVIEW: The Medical Social Worker provides emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. A few position highlights include: Assist the patient and family with health related financial, social and emotional concerns and assist with effective coping skills. Provide social work services in accordance with the plan of care. Ensure that changes in the patient's clinical status are communicated to other healthcare team members and the agency in a timely manner. Educate the patient and family members and other team members on medical social work interventions and treatment plan. Perform care planning, transfer and discharge functions. Identify risks or threats to the patient's health in the patient's environment and promptly implement appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient, the physician, and other members of the healthcare team to coordinate care effectively. THE MUST HAVES: Master's degree (MSW) or (BSW) from a school of Social Work accredited by the Council on Social Work Education. Must have a current and unencumbered Social Worker License in the state where services are rendered. Must have one (1) year of social work experience in a health care setting. Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation. Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
