Position Responsibilities

Perform physical therapy evaluations and create customized treatment plans Deliver high-quality therapy services to short-term rehab and long-term care residents Focus on improving strength, mobility, balance, and daily function Collaborate closely with nursing, therapy, and interdisciplinary teams Educate residents and families on treatment plans and progress Complete timely and accurate documentation

Qualifications

Active Physical Therapist license Skilled Nursing or geriatric therapy experience preferred Strong clinical and communication skills Positive, team-oriented attitude Passion for resident-centered care Compensation & Benefits:
