The Licensed Clinical Social Worker Social Worker will serve as a pivotal member of the team, providing therapeutic support and advocacy for clients facing complex personal and systemic challenges, including crisis intervention. This role requires exceptional empathy, strong clinical and organizational skills, and a proactive, crisis-responsive approach to problem-solving. The Social Worker will play a vital role in fostering client self-sufficiency, promoting dignity, and empowering individuals to lead fulfilling and independent lives, while also providing immediate support and de-escalation during times of crisis.

Duties & Responsibilities
Coordinate participant intake and enrollment for the Joyous Ventures program Provide direct support to participants, including intake assessments, case management, and crisis intervention.
Facilitate weekly life skills groups (health education, self-management coaching, overdose prevention training) and lead weekly house meetings.

Collaborate with participants, service providers, and the Joyous Ventures team to assess needs and develop treatment/service plans.
Conduct needs assessments (housing, mental health, substance use, medical care, social services) and develop individualized service plans (ISP) with clients, outlining SMART goals.
Connect participants to community-based services (behavioral health, substance use treatment, housing, food assistance, and transportation).
Cultivate a supportive community atmosphere and promptly address participant concerns.
Maintain comprehensive, accurate, and current participant records in electronic databases and charts - Client Management System (CMS), ensuring timely completion of Adult Initial Assessments, Discharges, and Progress Notes per Joyous Ventures and JCOD standards.
Apply harm reduction, motivational interviewing, and trauma-informed approaches to support participants and foster trust.
Encourage treatment adherence and assist in medication management support.
Meet weekly with participants, documenting case notes, assessments, referrals, and care plans in the CMS database within 48 hours of service delivery.
Maintain accurate records and generate reports on program activities, outcomes, and challenges.
Participate in weekly case conferences with DHS and clinical teams to discuss participant needs and challenges.
Maintain strict confidentiality and compliance with HIPAA and agency policies.
Attend required staff meetings, individual and group supervision, case conferences, and training sessions.
Contribute to the development and delivery of clinical training.
Advocate for participants' needs and the facility within the community.
Provide 24/7 on-call crisis intervention, de-escalation, and support.
