The Case Manager coordinates client care and develops individualized service plans to help individuals obtain housing stability and improve independent living skills. Responsibilities include maintaining clinical documentation, conducting home visits, and collaborating with a multi-disciplinary service team.
A bachelor's degree in social work, counseling, psychology, or a related field is required. Proficiency in Microsoft Office and experience working with marginalized communities or homelessness is preferred.
Primary responsibility of a Case Manager is coordinating all aspects of client care, support, and case plans to assist clients in obtaining/maintaining housing stability, preventing recidivism of homelessness, and improving overall independent living skills and quality of life. The Case Manager serves as an integral member of a multi-disciplinary, housing-focused service team. Please include a cover letter when submitting your application.
