Requires a high school diploma or equivalent and at least two years of social services experience. Candidates must be bilingual in English and Spanish and have experience working with populations facing mental health or substance use challenges.

This position is based in Lantern’s Supportive Housing program in Manhattan, serving formerly homeless families living with chronic physical illness and/or mental health challenges.
We are seeking an experienced Bilingual Case Manager to support formerly homeless families in stabilizing their lives, maintaining housing stability, and achieving their physical, emotional, vocational, and social goals. The Bilingual Case Manager will assess client needs, develop service plans, connect residents to appropriate services, monitor progress, and maintain thorough documentation. The ideal candidate is passionate about helping people, demonstrates strong clinical judgment, and can work both independently and as part of an interdisciplinary team. This individual will have experience supporting individuals impacted by mental health challenges, substance use, legal involvement, and chronic medical conditions.
This position operates in a supportive housing site in Manhattan and works with individuals who may experience mental illness, substance use, disability, HIV/AIDS, and/or other chronic health conditions.
While performing the duties of this job, the employee is regularly required to stand, walk, sit, stoop, climb stairs, and/or kneel. Occasionally, the employee may lift and/or move up to 10 pounds.
