• Range: $120,000.00 SalaryCome join our team as a full-time Family Nurse Practitioner (FNP) at Community Health Services- 2221 Hayes Avenue, Fremont! This is an outstanding opportunity for an experienced, compassionate FNP, who enjoys caring for a diverse population of patients--including both adults and children. Applicants must have at least 3 years of experience in a family practice setting. $120,000 base salary, no RVU's and paid malpractice insurance. As a Federally Qualified Health Center, our providers are eligible to apply for student loan repayment through National Health Service Corps (NHSC)- up to $75,000 for full-time employees. Other benefits include paid vacation, sick time, and retirement benefits.

• You’ll Do: ✅ Work directly with patients Monday through Friday- No Weekends ✅ Perform Adult and Pediatric Assessments ✅ Communicate clearly with Providers, Patients, and CHS Staff ✅ Follow-up on Lab and Diagnostic testing ✅ Administer injections as needed ✅ Follow all safety,

• For: ✅ Graduate of a current accredited Family Nurse Practitioner (FNP) Program ✅ Current License to practice in the State of Ohio as a Family Nurse Practitioner (FNP) ✅ Current Basic Life Support Certification ✅ Minimum of three years as an FNP preferred ✅ Knowledge of documenting with EHR ✅ Strong communication skills and the ability to multi-task ✅ Ability to critically think and problem solve ✅ Reliable, professional, and compassionate
