Locations
Provide primary, urgent, and preventive care to residents in skilled nursing and assisted living facilities. Manage acute and chronic conditions while collaborating with physicians and nursing staff to coordinate patient care.
Requires an active and unrestricted Nurse Practitioner license in Tennessee and national board certification (AANP or ANCC). Preference is given to candidates with 1-2 years of experience in primary, geriatric, or long-term care settings.

As a Nurse Practitioner with Ultra Health Providers, you will serve as a trusted clinician for residents in your assigned facility. You’ll manage acute and chronic conditions, provide preventive care, and partner closely with nursing staff and administrators to ensure exceptional patient outcomes.
