Acts as the primary liaison between the hospice organization and hospital systems to coordinate care for patients. Responsible for conducting nursing assessments, managing admissions, and collaborating with discharge planners to ensure seamless transitions of care.
Requires a Diploma or Associate's Degree in Nursing with a current NC/SC Registered Nurse license and CPR certification. Candidates must have at least 2 years of related experience in direct care for patients with life-limiting illnesses.
