Provide administrative and clinical support to a care team serving patients with serious long-term illnesses. Responsibilities include managing EHR documentation, coordinating patient outreach, and serving as a communication hub between providers and families.
Must be a graduate of an accredited Medical Assistant program with current MA and BLS certifications. Experience in palliative or chronic care and proficiency with EHR systems is preferred.

Qualifications Graduate of an accredited Medical Assistant program; current MA certification required (AAMA, AMT, NHA). Current Basic Life Support (BLS) certification. Experience in palliative, hospice, or chronic care preferred. Strong communication, organization, and problem-solving skills; empathetic and calm under pressure. Proficiency with Electronic Health Records (EHR) and virtual health technologies **Experience with Home Care Home Base is strongly preferred Key Responsibilities: Administrative Support Maintain accurate and timely documentation in the electronic health record (EHR), ensuring patient information and care coordination activities are properly recorded. Conduct outreach to patients with chronic conditions via phone, email, or approved virtual platforms to support follow-ups, care plan engagement, and appointment reminders. Serve as a communication hub between patients, families, providers, and community partners to ensure timely coordination of care and information sharing. Support provider and care team workflows by assisting with scheduling, calendar coordination, and tracking follow-up tasks or referrals. Answer and route incoming calls, assist with patient inquiries, and help maintain a professional and responsive front office environment. Assist with scheduling appointments and managing patient calendars to support efficient clinic operations. Monitor office and clinical supply levels, coordinate ordering, and maintain organized inventory records.
