The LPN is responsible for managing and administering medications for PACE participants and implementing primary care orders. They coordinate with physicians, pharmacies, and caregivers to ensure accurate medication schedules and care transitions.
Candidates must hold a current Kentucky Practical Nurses License (LPN) and CPR certification. A minimum of one year of experience working with the frail or elderly population and a valid driver's license are required.

Senior Community Care of Jefferson County is part of Volunteers of America National Services which serves as the Housing and Healthcare affiliate of the Volunteers of America parent organization.
$28.10 - $33.30 based on experience.
Monday - Friday, with occasional weekends
On Call Rotation, is required.
The LPN is responsible for the effective management and provision participant medications provided via the PACE medication department. Administers medications, implements primary care orders, and evaluates participant needs physically and/or by telephone. Provides education to Senior CommUnity Care participant and caregiver in regards to plan of care and medications. Communicates with participant’s primary care givers, physician, clinic staff and interdisciplinary team.
