• Key Responsibilities Perform comprehensive health assessments and physical examinations in patients' homes.
• Diagnose and manage acute and chronic health conditions.
• Develop and implement individualized care plans.
• Prescribe medications and treatments within the NP scope of practice.

• Provide patient and family education on health maintenance and disease management.

Qualifications

Education: Master of Science in Nursing (MSN) or higher from an accredited Nurse Practitioner program.

Licensure

Active New York State Nurse Practitioner License (Required).

Certification
