$28 - $35 / HOUR
Provide compassionate hospice care by assessing patients' physical and emotional conditions and reporting changes to the interdisciplinary team. Collaborate with RN Case Managers to implement and revise care plans to ensure patient comfort and family support.
Requires a current Maryland LPN license and at least 2 years of experience in med/surg or home care. Must possess a valid driver's license, reliable transportation, and comply with vaccination requirements.

Qualifications Your Qualifications: Current Maryland LPN license and thorough knowledge of nursing principles. 2 or more years of nursing experience in med/surg or home care required, with experience in hospice, end-of-life, or critical care experience preferred. Familiarity and comfort with documenting in an Electronic Medical Record. Valid driver’s license and acceptable driving record and reliable transportation Ability to meet the physical demands of the position with or without reasonable accommodation.
