The Primary Care Nurse is responsible for assessing, directing, and coordinating patient care and clinical procedures. They serve as patient advocates and health educators while supporting providers in a fast-paced environment.
Candidates must be graduates of an accredited School of Nursing with a current New York State RN license. BLS certification is required within 3 months of hire, and a BSN is preferred.

The Primary Care Nurse is responsible for assessing, directing, and coordinating patient care and clinical procedures in an efficient, empathetic, and timely manner appropriate to the age of patients served. This role also serves as a patient advocate and health educator while supporting providers and care teams in a fast-paced primary care environment.
Assess and coordinate patient care and clinical procedures Deliver compassionate, patient-centered nursing care Educate patients and families on health conditions and treatment plans Advocate for patient needs and promote positive health outcomes Collaborate with providers and interdisciplinary healthcare teams Maintain accurate and timely documentation
