The Nurse Practitioner is responsible for providing high-quality primary care in a person-centered care model, diagnosing and managing both acute and chronic conditions. They collaborate with other healthcare professionals and involve residents and families in care planning.
Candidates must have a Master of Science in Nursing or equivalent, national certification, and an active state license. A minimum of 2 years of clinical experience in a relevant setting is required, with a preference for experience in geriatrics.
