Provides 24/7/365 crisis intervention services via telephone, face-to-face, and telehealth interactions. Acts as a community liaison between the Northeastern Center and referral sources like hospitals and nursing homes.
Requires a Bachelor's Degree in a Mental Health related field and lived experience with mental health or substance use recovery. Must have a valid driver's license and obtain a Certified Peer Support Professional Certification within 90 days.
The Emergency Solutions Case Facilitator operates as a team member under the supervision of the Emergency Solutions Team Manager.
Primary job duties include 24/7/365 on-call for crisis intervention services and acting as community liaison between Northeastern Center and referral sources. On call services include telephone, face-to-face, and telehealth interactions with clients. Follow-up action on previous emergency calls and maintaining positive working relationships with referral sources (hospitals, nursing homes, etc.) are other essential tasks.
Bachelor’s Degree related to Mental Health; This position requires the applicant to have lived experience, defined as either (1) personal experience in their own recovery from a mental health condition or substance use disorder, or (2) direct experience supporting a loved one in their recovery journey.
Must possess a valid state issued drivers license.
Certified Peer Support Professional Certification (Must be obtained within 90 days of hire).
Shift: First Shift, Full time, 6a-2p (M-F), alternating weekends on-call
