The Community Health Worker (CHW) plays a vital role in supporting members of Select Health by addressing social needs that impact health outcomes. As a trusted community liaison, the CHW partners with care managers and health plan staff to engage, educate, advocate, and coordinate services for individuals with complex health and social challenges. This role includes outreach in home and community settings and contributes to integrated care planning and delivery. Essential Functions Maintain regular contact with members through phone, digital platforms, home visits, and community outreach. Collaborate with care managers and health plan staff to support coordinated care and transitions. Participate in interdisciplinary team meetings to contribute insights on member needs and community resources. Maintain a comprehensive and current knowledge base of local resources and community partnerships. Maintain accurate and timely documentation in care management systems.
High school diploma or equivalent. Strong communication skills (verbal, written, and listening). Ability to work independently and collaboratively in clinical and community settings. Willingness and ability to conduct home and community visits. Basic computer proficiency and comfort with electronic documentation. Valid driver’s license, insurance, reliable transportation, and acceptable driving record. Experience working with diverse populations, including individuals with limited financial resources, low health literacy, or communication barriers. Active Community Health Worker (CHW) Certification Preferred Qualifications 1-3 years as a Certified Community Health Worker (CHW) Familiarity with managed care principles and health plan operations. Bilingual proficiency (e.g., English/Spanish or other relevant languages). Physical Requirements Ability to read, write, and communicate effectively in person and via technology. Frequent interaction with members, providers, and team members requiring verbal communication and active listening. Manual dexterity for computer use and documentation. Ability to drive and navigate community settings safely.
