The Transition Specialist assists individuals and families in transitioning from institutions to community living by coordinating services and discharge planning. They collaborate with staff and agencies to resolve barriers and document all service provision in the organization's database.
A high school diploma is required, though a college degree is preferred, along with a valid driver's license and dependable transportation. Knowledge of disability systems, entitlement programs, and the ADA is strongly preferred.
