The Social Services Designee coordinates the overall interdisciplinary and psychosocial care plans for residents from admission to discharge. They act as a primary liaison between residents, families, and healthcare personnel to ensure effective care delivery.
Candidates should have related coursework in social work or psychology and the ability to communicate effectively in English. Experience in a hospital or nursing home setting is preferred.

The Social Services Designee coordinates the overall psychosocial care plan for residents. The Social Services Designee provides support and advocates for resident/patient and their family. The Social Services Designee will develop and maintain community resources for residents.
