The Nurse Practitioner performs palliative care consultations for inpatient and outpatient settings, including home visits. They collaborate with physicians to develop care plans, write medical orders, and provide education to staff and families.
Candidates must be a Family or Critical Care Nurse Practitioner licensed in South Carolina. Certification in Palliative Care is required or must be obtained within 12 months of qualification.
Family or Critical Care Nurse Practitioner licensed in SC Certification or intent to complete certification within 12 months or upon qualification as Palliative Care Nurse Practitioner Experience in the care of patients with advanced illness. Duties/ Responsibilities Serves as member of the Supportive Care Service Team in the hospital Completes palliative care consults on referred patients, collaborates with physicians and makes recommendations for care including writing medical orders according to hospital policy Available as resource for patients, families and staff regarding palliative care and end of life care Participates in regularly scheduled conferences of Palliative Care Team Attends patient care conferences on unit, clinic, and home visits as indicated Assists with in-services and education for physicians and hospital staff on Palliative Care Collaborates and assists hospital case managers with discharge planning on referred patients Provides required documentation of services provided and submits timely and accurate information used for billing Provides staffing and does palliative consults for patients referred to Supportive Care Clinic/home visits May provide consults as assigned in outpatient Palliative Care Clinic/home visits In addition to Hospice and Palliative Care of the Piedmont; adheres to policies of Self Regional Healthcare Provides and completes all required documentation for billing as well as data collection Supports the overall culture of the organizations Participates in organization sponsored events Performs related duties as assigned or as needs arise Hospice and Palliative Care of the Piedmont is an Equal Opportunity Employer
