The Physical Therapist provides professional physical therapy services to clients of all ages, planning and implementing treatment programs tailored to individual needs. They regularly re-evaluate care plans and communicate with patients and healthcare teams to ensure effective care.
Candidates must have a degree from an APTA accredited program and be licensed as a Physical Therapist in New Hampshire. A minimum of one year of experience in a healthcare setting is preferred, along with BLS and CPR certifications.

About the Job: Provides professional physical therapy services to clients/patients of all ages and families whose needs change from basic to complex and are rapidly changing. Plans and implements age-appropriate specific treatment programs for individual client/patients with a level of skill in accordance with accepted principles and practice. What You’ll Do: Plans, develops, and implements physical therapy care plan appropriately. Regularly re-evaluates and updates appropriateness of care plan and physicians’ orders. Communicates appropriate information to patients, physicians, peers, managers, physicians, departments, and other agencies involved. Integrates problem solving methodology and quality improvement in resolving problems related to patient care and organizational processes. Orients and supervises PTA’s according to State of NH guidelines and VNA policy. Who You Are: Education: Degree from an American Physical Therapy Association (APTA) accredited program required.
Participate in rotating weekend commitment.
