Role: The Physical Therapist is responsible for evaluating, planning, and carrying out individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical team.
Key
Responsibilities:
Provide effective patient care using approved therapy techniques and procedures
Evaluate patient condition and establish measurable treatment goals
Develop and implement individualized treatment plans
Supervise and coordinate care with PTAs and HHAs
Document patient progress, updates, and treatment notes accurately and on time
Educate patients and families on exercises, mobility, and use of adaptive equipment
Evaluate home environments and recommend modifications to improve safety and independence
Ensure all services meet HIPAA standards for confidentiality
Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care
QualificationsQualifications:
Active Physical Therapist (PT) license in CA
Minimum 1 year of PT experience preferred
Physical exam within the last 6 months
Current TB test (within 12 months) and CPR certification
Valid driver’s license and reliable transportation
