Salary
$90,000 - $110,000 / YEAR
Evaluate patients in their homes to develop and deliver individualized physical therapy treatment plans. Coordinate with clinical support teams and maintain accurate documentation to improve patient mobility and independence.
Must possess an active New Jersey Physical Therapist License and current CPR certification. Experience with Medicare documentation and billing is preferred, along with the ability to work independently.

As a Home Care Physical Therapist, you’ll work directly with patients in their homes and communities to help them regain mobility, improve function, and maintain independence.
