You will be directly\nworking with patients, following a one-on-one care plan in patient homes. \nYou operate with a big heart, not only providing personalized and attentive\ncare, but by building genuine relationships with those you serve. You are\ndetail-oriented, highly organized, and committed to creating a healthy\nenvironment that meets patient needs. \nStrong verbal and written communication skills are needed to succeed in this\nrole. Prior Home Health Care experience a plus.\n\nResponsibilities: Monitoring Safety (FALL RISK) and Companionship\n(Assist as needed) with medication reminder, errands and light housekeeping.\n \nFulltime position\nSix (6) hours daily 9am to 3pm / (42) hours weekly\n\n**Seeking a mature/35 or older caregiver for the client**\nNon-Smoker
