The Registered Nurse is responsible for planning and directing hospice care, including conducting in-home visits and completing patient evaluations. They develop personalized care plans and administer prescribed medications and treatments while communicating patient needs to the care team.
Candidates must hold a valid RN license and a current CPR certification. A valid driver's license with an insured vehicle is required, and experience in community health or hospice is preferred.
Completing hospice evaluations as requested (admission if comfortable and training would be provided) Conducting in home visits as assigned; assessing the patient’s condition, and teaching the patient and family Developing care plans based on diagnosis and needs Administering medications and treatments as prescribed by physician Communicating patient needs and changes with the team as necessary
Must hold a current, valid RN license through the State Board of Nursing Community health/hospice is preferred but willing to train the right candidate. Complies with accepted professional standards and practice. Demonstrates good verbal and written communication, and organization skills. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Possesses and maintains current CPR certification.
