The nurse provides telephonic and in-person risk assessments and crisis intervention for potential patients. They determine the appropriate level of care and maintain communication between referral sources, families, and unit staff.
Requires a valid Registered Nurse license from the Tennessee Board of Nurse Examiners and a relevant degree in nursing or behavioral health. Prior experience in emergency departments, intake, or psychiatric crisis intervention is preferred.
The Triage and Assessment Counselor/Nurse provides telephonic and in-person risk assessment and crisis intervention, initial contact with referral sources and potential patients, determines appropriate level of care and maintains communication with referral sources, families, patients and unit staff. Triage and Assessment Counselor/Nurse receives individual/family walk-ins with compassion, empathy, a sense of urgency, and to ensure each person receives an appropriate assessment. Thorough and accurate documentation is a critical component of the job.
