• What You’ll Do & Key Responsibilities: Support residents and families with emotional, psychosocial, and adjustment needs during their stay.
• Coordinate care planning and collaborate with interdisciplinary teams to support resident goals and well-being.
• Assist with admissions, discharges, and transition planning to ensure continuity of care.
• Connect residents and families with community resources, support services, and financial assistance programs as appropriate.
• Maintain accurate documentation and ensure compliance with federal, state, and facility requirements.

Qualifications

Bachelor’s degree in social work, Gerontology, or a related Health and Human Services field. Ability to obtain or maintain a Licensed Social Worker (LSW) credential. Strong interpersonal and communication skills with the ability to build positive relationships and communicate effectively.
